The website I chose with a form was from my very own place of employment, University of Wisconsin – Stevens Point (UWSP), where people can submit a form requesting information. The different input types from what I can tell are text, email, textarea, select (dropdown) menu, checkboxes, and a submit button.

  1. When I went to validate the website, it showed 30 errors, 5 warnings, and 73 info messages.
  2. I decided to click on the “Contact Us” in the upper-right navigational menu. When I went to validate this page, it showed 21 errors, 5 warnings, and 100 info messages.
  3. The first page I went to validate (with the form) had 30 errors, 5 warnings, and 73 info messages and the second page I went to validate (contact us page) had 21 errors, 5 warnings, and 100 info messages.
  4. Given the two pages I looked at I rate the site’s design a 9. The initial form I viewed was very simple to understand. It asked for helpful information given its purpose. The contact us page for the Admissions Office used good visuals with images, the map image, and each person’s contact information box. I also thought it was neat how the form on this page showed up as each question was answered. It was good it was short though, or I may have wondered how long it went!
  5. Given the two pages I looked at I rate the usability 9. The initial form was very easy to use. I did not find it too long. A reset button would have been helpful if you wanted to start it over. The contact us page for the Admissions Office had a helpful form that helped to narrow down the best person to contact, while still displaying all the Admissions Office staff. However, it is a lot to scroll down to view the information, which is a theme on many pages for the UWSP website. I do not find that helpful as I often need to look up information quickly.
  6. The theme for the two web pages I looked at was to gather information or who to contact for more information. I can confirm that these pages met their purpose, as long as there is follow-up from the university due to the submission of the form, as well as when contacting the Admission Office’s staff.
